ADMINISTRATIVE GUIDELINE FOR APPLICATION OF REMARKING / REVIEWING FOR SPM/SPM(U)/STAM
1. Application must be made within 30 days of the formal announcement of the result using Borang Semak Semula/Kes T LP/Am 65 Pin. 1/2012 which is available in schools, Sektor/Unit Penilaian Dan Peperiksaan, Jabatan Pelajaran Negeri, Semak Semula Application counter at ground floor Lembaga Peperiksaan Putrajaya or downloaded from http://www.moe.gov.my/lp 2. Incomplete form will not be processed. (ii)Application form for SPM(U) must be submitted with a certified true copy of SPM(U) result slip and school based assessment report. (iii)Application form for STAM must be submitted with a certified true copy of STAM result slip, marks and grades for STAM trial. All copies must be certified by School Principle or Division 1 Government Officer. 4. Processing fee of RM50.00 is chargable for each subject using money order/postal order/bank draft made out to Pengarah Peperiksaan. Cash and personal cheque are not acceptable. 5. Application form should be enclosed with 2 self-addressed envelopes (11 cm x 22 cm) bearing 60 sen stamps for each envelope. 6. Completed application form with supporting document and payment should be addressed to: Please indicate SEMAK SEMULA SPM or SEMAK SEMULA SPM(U) or SEMAK SEMULA STAM at the top left hand corner of the envelope. 7. Application form received after 30 days will not be processed. 8. Official result will be posted within 2 months of the closing date of the application.

Application form should be enclosed with 2 self-addressed envelopes (11 cm x 22 cm) bearing 60 sen stamps for each envelope. 6. Completed application form with supporting document should be addressed to: Please indicate KES T SPM or KES T SPM(U) or KES T STAM at the top left hand corner of the envelope. 7. Application form received after 30 days will not be processed. 8. Official result will be posted within 2 months of the closing date of the application. |
